Left out or changed by Shishya: SPARE-LIST SUBSTITUTE (replaces jnanpith-award). Source is MoSPI, not NITI Aayog: the old Planning Commission plan-index pages on niti.gov.in return 404, and no NITI page listing all plan periods was found. All 12 plans plus the 'Three Annual Plans' (1966-69, the 'plan holiday') and the 'Rolling Plan' (1978-80) printed in the chapter are included. Left out: the Annual Plans 1990-91 and 1991-92, which the chapter mentions only in running text, not as a plan heading. Periods kept as printed (spacing varies). The chapter prints no as-on date; its text says five-year planning is set to change with NITI Aayog (NITI's Annual Report 2016-17 confirms the five-year-plan era ended with the Twelfth Plan in March 2017).
Names and spellings are kept as the source prints them. Spotted a change on the official page? The list is re-read from the source, never from other sites.
